Conservative treatment with metal band for molar-incisor hypomineralization: A case report

Molar-incisor hypomineralization (MIH) is a systemic enamel defect that affects one to four permanent first molars and is frequently associated with incisors. Its etiology is multifactorial and is linked to an increased risk of dental caries, hypersensitivity, aesthetic concerns, restoration failure, and, in some cases, the need for extractions for orthodontic purposes. This case report aimed to document a conservative treatment involving the placement of a metal band on a molar affected by MIH with post-eruptive enamel breakdown (PEB). The treated tooth was classified under criterion 5 of the MIH Severity Scoring System (HMI-SSS), corresponding to PEB with exposed dentin that was hard to touch. An orthodontic band was cemented using type I glass ionomer cement (GIC), and a high-density, fast setting encapsulated restorative GIC was used to reconstruct the lost tooth structure. Clinical follow-ups were conducted at two, four, and six months. At the final check-up, only minimal loss of occlusal GIC was observed, with no alterations to the band or surrounding tissues. In conclusion, this technique proved to be an effective, accessible, and clinically viable option for managing molars with PEB. However, further studies are recommended to assess its long-term durability.
